Remote Patient Monitoring During COVID-19: An Unexpected Patient Safety Benefit

The authors note that “A recent cost-utility analysis estimated that daily assessment [by the clinical monitoring team] and 3-week follow-up of at-home pulse oximetry monitoring was projected to be potentially associated with a mortality rate of 6 per 1000 patients with COVID-19, compared with 26 per 1000 without at-home monitoring. Based on a hypothetical cohort of 3,100 patients, the study projected that remote monitoring could potentially be associated with 87% fewer hospitalizations, 77% fewer deaths, reduced per-patient costs of $11,472 over standard care, and gains of 0.013 quality-adjusted life-years.” Masimo SafetyNet and Radius PPG® with SET® pulse oximetry were used in the study.
Case Example
Impact of Remote Patient Monitoring on Length of Stay for Patients with COVID-19
Journal of Telemedicine and e-Health
Researchers evaluated the impact of remote patient monitoring of COVID-19 patients on hospital length of stay (LOS), hospital readmission rate, and 30-day-return-to-ED rate. To do so, they compared outcomes for 1,979 patients discharged with (n = 203) and without (n = 1776) Masimo SafetyNet with Radius PPG tetherless pulse oximetry, enrolled over two time periods. They found a significant association in the second phase between decreased LOS and patients discharged with Masimo SafetyNet without home oxygen therapy (compared to non-Masimo SafetyNet patients). They also found a “trend toward an overall lower LOS” for patients discharged with Masimo SafetyNet, without an increase in ED returns or hospital readmissions, although the association was not significant. The researchers noted that patients enrolled in RPM had a higher proportion of 30-day ED visits, which they suspect is due to early escalation of care for patients who are being closely monitored, compared to patients who are not under such scrutiny. They concluded, “Home telemonitoring after discharge for patients with COVID-19 may be a safe tool that may reduce the mean duration of hospitalization and create more bed capacity.”
Case Example
In a study evaluating the impact of Masimo SafetyNet® and a hospital’s virtual response center (VRC) on 30-day readmission rates for 100 patients undergoing acute postoperative recovery after total joint arthroplasty (TJA), researchers found that patients discharged with Masimo SafetyNet (n=50) had significantly lower rates of hospital readmission and ED visits than patients discharged with routine instructions. The researchers also calculated that use of the remote patient management program could potentially save $800,000 for every 1,000 TJA surgeries performed, and that most patients held a favorable view of the program. The authors concluded, “Remote home monitoring with a virtual response team after outpatient TJA is a feasible way to mitigate readmissions in the acute postoperative period and increase patient satisfaction.”
